We’re excited to see you at 3Dimensional Physical Therapy. Please follow the instructions on this page to make your first visit as efficient as possible. We recommend you fill out the NEW PATIENT FORM before arriving at your first appointment.

For your first visit:
• Please arrive 10 minutes early to complete necessary paperwork. To save time, please download and complete our NEW PATIENT FORM and bring it with you.
• Remember to bring your insurance card, medical coupon, or claim information and referral form from your doctor (if applicable).
• Bring a change of comfortable clothes that allow easy movement for your injury.
• The first session will last at least one hour, proceeding visits should last forty-five minutes to one hour. Your past medical and current injury history will be recorded and a physical examination will be performed to determine the nature and severity of the injury.

Insurance
Patients may be seen at 3Dimensional Physical Therapy with or without a written referral for physical therapy. Legally, you get to choose your own physical therapist. However, many insurance companies do require a referral and/or pre-authorization for payment of physical therapy service. We are in-network with a broad range of insurance companies, including but not limited to:

If you prefer to verify your own coverage with your insurance company:
• Contact your insurance company to confirm physical therapy coverage.
• Determine your insurance: Yearly dollar coverage limits, number of approved treatment visits for therapy, and need for pre-authorization of physical therapy treatment sessions.
• Record the name of the insurance company’s representative.
• Record any pre-authorization numbers provided by the representative.
